| Safe administration of medication is of utmost importance in the school’s Health Office. The following information is designed to inform parents about medication administration guidelines and expectations at school. Permission Requirements All requests for medication administration require a parent’s signature. Prescription medication must also include a physician’s signature. The permission form covers the current school year and will need to be renewed each fall. Continued use of an over-the-counter medication may be reviewed and physician permission required. Chemical Free School Our school is a chemical free school. Students requiring medication, whether prescription or over-the-counter need to keep them in the Health Office. Students requiring immediate access to certain medications may be allowed to self-administer with parental and/or physician permission. Self-administration requires a student/nurse agreement (addresses school policies, safety, dosing and medication teaching). Transporting medications to school For the safety of all students, we require that parent’s bring their child’s medication to the Health Office directly. If this absolutely cannot be done, we require a phone call to the Attendance Hotline (651) 345-4528 Bluff View or (651) 345-4472 Lincoln High alerting staff to the type of medication and number of tablets sent on the day the medication was sent with a student. The container needs to be secured and tamper resistant and delivered to the Health Office immediately upon arrival of the student to school. Medication Containers All medications (both prescription and over-the-counter) need to be in the original container labeled by the manufacturer or the pharmacy. Elementary students need to have pediatric dosage guidelines relating to their weight &/or age on over-the-counter medications. The health office requires physician permission to administer medication outside the recommended labeled dose for both elementary and high school students. Aspirin or aspirin containing products will only be administered upon written direction of a physician, because of the risk of Reye’s Syndrome. Notification of need for re-fill’s It is the parent’s responsibility to maintain an adequate supply of their child’s medication. We will attempt to notify parents via note or phone when medications are running low. Medication Changes or discontinuation It is the parent’s responsibility to provide a written statement to notify the Health Office of 1. a desire to discontinue medication administration at school 2. a prescription medication or dosage change (signed by the physician and parent) End of the Year The school will not send ANY medication home with students. At the end of the year parents will be notified of the amount of medication remaining. Parent’s can arrange pick-up or disposal of medications. Any medication not picked up by the last day of school will be disposed of. Field Trips and Off-campus activities Medication administration is delegated to a teacher or other staff member for off-campus activities. Prescription medications, labeled for each student along with a first aid kit are sent on all field trips. Over-the-counter medications are not routinely sent on field trips. Please contact the Health Office if you feel your child’s O-T-C medication needs to be sent on field trips. All student’s who self-administer/self-carry their medications are responsible for bringing them to all off campus activities. District Nurses The district currently employs one full time Licensed School Nurse (BSN, RN) and one ¾ time Licensed Practical Nurse (LPN) to staff the Health Offices of both buildings. Designated office staff covers the Health Office when the nurses are unavailable.
